The SPD pictured is not to be confused with another less-detailed booklet entitled "Your New Cash Balance Account" that AT&T distributed in November, 1997. AT&T mailed the SPD to employees in January, 1998 using bulk rate mail. It has been established that some employees received it while others did not. There is no right or wrong answer to the survey. Just answer the question truthfully to the best of your knowledge.
